Page 1 of the Direct Deposit Form
- Includes the Employer Name, Client ID, and Employee Name.
- Select the type of account, Checking or Savings.
- The form has three individual account sections if sequencing needs to occur.
- Branch, City, State, and Zip are additional fields for the form only.

- Contains a blank section for a copy of the voided check, bank letter, or deposit ticket.
- Has a section for the employee to sign, print, and date the form.
